Dr. Choi is an expert in diagnostic imaging, with an emphasis in spine and musculoskeletal imaging, including plain radiographs, MRI, and CT.  Currently Dr. Choi is licensed in Illinois. He provides:
  • Detailed reports within 24 hours
  • Preliminary reads and STAT service on emergency cases
  • A quick and easy process to receive imaging consultation if you have digital X-ray (Please leave your emailCONTACT information for further information)  
  • Consultation regarding patient care in relation to radiographic findings
  • Discussion of necessity of further imaging
  • Quality control and troubleshooting for your X-ray suite.

Instructions for imaging consultation (Depending on location local pick-ups can be provided) 

  • Fill out the radiologic consultation request form, which is available at the bottom of this page.
  • Make sure the information on ID blocker on the radiographs is legible.
  • Enclose the consultation request form and radiographs in mailing jacket

  • You can send the package via USPS (Other carriers such as UPS and FedEx will not deliver to p.o. box). 

Plain radiographs per body part/section or study $25
MRI/CT per body part/section or study $50

  • PATIENT CONSENT FORM FOR BILLING INSURANCE COMPANY - If you want us to bill the referring doctor, this form is not necessary

What you can expect from our service:
  • Upon receipt of images, a detailed diagnostic imaging report will be generated, covering postural alterations, pathology, pertinent negative findings, and recommended further procedures/studies including advanced imaging, appropriate laboratory studies, and referrals. 
  • Should there be significant pathology or contraindication for treatment including high velocity low amplitude (HVLA) adjustments, preliminary reporting via phone call will be given immediately.
  • Short turnaround time for diagnostic imaging reports (<24 hours).
